How do I get started with Myhorizon?

Can I make an enquiry for a family member?

Yes. Family members, friends and support workers can all make an enquiry for someone else. Tell us who the support is for and what they're looking for, and we'll talk it through with you.

Can a support coordinator refer someone to Myhorizon?

Yes. Support coordinators, plan managers and other professionals can refer someone to us. Call us on 1300 694 674 or make an enquiry online, and tell us which service you're referring them for.

Can I move to Myhorizon from another provider?

Yes. You can choose and change your NDIS providers. Check your current service agreement for its notice or cancellation period, then call us on 1300 694 674 and we’ll talk through the support you need.
